Like in Crusader Kings 3, Hearts of Iron IV and historical functionality there are also game mechanics which allow players/AI to create new states and thus allow to play a more accurate history or a more alternate history.

How to form nation?
To form one of nation which I mentioned above you. need to open Cultures > National Foundation where you could see which states you could form and how much of progress you have.
After selection you could find details of nation foundations which is usually limited by how much states you country in specific nation owns. Bellow you could see that to form Iberia I need 17 states from 19 and now I have 12 of 19. So If I need to found Iberia nation I need to conquer Portugal.
To form nation you doesn’t need to conquer all states you could own some states with dominions, personal unions, puppets or vassals. In this case you will get states which are theyr and you need it to form nation to your ownership.
There are also more “complicated” formation of nations which works in case Germany, Italy and Scandinavia. In which you form nation with journal entries and diplomatic plays. So if you play and you need to form this nation then you follow your journal.
